Close friend found his body
Tragedy struck early Thursday morning when actor Michael Madsen was found unresponsive in his Malibu home. The news sent shockwaves through Hollywood — and now, we’re learning what may have caused the beloved actor’s sudden passing.
According to his longtime manager Ron Smith, Madsen suffered a fatal cardiac arrest. He confirmed the heartbreaking cause of death to NBC News, while also telling Daily Mail that the 67-year-old star’s life ended far too soon.

Dougie Smith says he hadn’t heard from Michael all day Wednesday, which felt unusual — so he stopped by to check in. What he found the next morning around 8:30 a.m. was heartbreaking: Michael was unresponsive in bed.
Fell off a horse two years ago
Just the night before, Dougie says they had spoken, and the Reservoir Dogs star sounded upbeat and excited — especially about heading to Nashville Comic-Con later that week. Nothing seemed out of the ordinary.
Michael Madsen, 67, had been dealing with lingering health issues stemming from a fall off a horse nearly two years ago. While surgery was on the table, the Kill Bill star decided to hold off for now, opting instead to wear a brace so he could stick to his busy schedule.
According to his friend and assistant Dougie Smith, Michael had gone in for a routine hospital visit at UCLA just last Friday to have his rotator cuff checked. Doctors also ran a PET scan to monitor a minor brain bleed he had suffered from the same fall. Dougie says the results came back clear, and everything looked fine.
“He looked like he had been in the best health he had been in all year!” Dougie told TMZ.
Long-term battle against alcoholism
At the same time, other reports contradict Dougie’s statements, revealing that Madsen had been struggling with a long-term battle against alcoholism.
Madsen’s longtime attorney Perry Wander, who represented the actor for 20 years, confirmed the heartbreaking truth: “Michael was suffering from the effects of alcoholism. He had multiple stints in and out of rehab. He struggled to maintain his sobriety. He was not happy about his life,” she told Daily Mail.
Just two days before the star was found dead, Wander spoke with him and sensed something wasn’t right. “I knew he was not well,” the attorney told Daily Mail.

Adding to the pressure was a bitter legal dispute with Madsen’s estranged wife.
According to Wander, the couple were locked in a painful battle over child support and financial issues. The attorney claimed the conflict escalated so much that “his passport [was] maliciously revoked,” which severely limited his ability to work overseas.
“I blame her for putting in the screws over his last years of life,” Wander said. “Michael lived a life of regrets – those regrets being his two marriages.”
Married three times
Though he was married three times, most recently to DeAnna Morgan, Madsen leaves behind four children. One of his sons, Christian Madsen, followed him into acting. Another son, Hudson, tragically died in 2022.
Despite his private pain, Madsen kept working. Over a career that spanned more than 40 years, he racked up an impressive filmography, not only appearing in Tarantino’s films but also showing up in fan favorites like Free Willy, Sin City, and Die Another Day, part of the James Bond franchise.
He may have often played the bad guy, but for fans, Madsen will forever be remembered as a cinematic legend — one who brought raw grit, pain, and power to every role.
