Presley Gerber’s death at just 27 has left his famous family and fans struggling to make sense of a life that, from the outside, seemed almost impossibly glamorous.
The son of Cindy Crawford and Rande Gerber died on September 20, 2026, at a rehabilitation facility in California, according to the Los Angeles County Medical Examiner.
His cause of death has not yet been determined, with the autopsy still pending.
Just days earlier, Presley had reportedly checked himself into treatment.
The devastating detail has cast an even darker shadow over the years of struggles he had increasingly spoken about himself; from addiction and mental health battles to treatment, relapse and repeated attempts to find a way forward.

But long before Presley was publicly discussing those struggles, there was one moment that caught the world’s attention for a very different reason.
At just 20 years old, the model made a shocking decision that seemed almost unthinkable for someone whose face was part of his career.
He had a single word permanently tattooed across his cheek.
The choice sparked an immediate backlash online. People questioned what he was thinking, while others focused on the reaction it might provoke from his famous parents.
But Presley had his own explanation for why he chose those particular words.
And looking back now, after everything he would go on to reveal about his life, the meaning behind that tattoo is difficult to overlook.
The tattoo that shocked everyone
In February 2020, Presley documented himself getting the unusual face tattoo in an Instagram video.

His sister Kaia later praised him for doing exactly that.

She said she was “deeply proud of him.”

Presley later revealed that he had been struggling with severe opioid addiction for years and had survived a near-fatal fentanyl overdose that left him in a coma.
In a since-deleted video, he spoke about addiction, panic attacks, psychiatric treatment and the medications he had been taking.

“I need some direction.”
And he kept searching for it.
The final months
In March 2026, Presley traveled to Mexico for an ibogaine “flood dose” as he pursued another possible path toward recovery.
Afterward, he described confronting things he had spent years running from.
“Letting go of control didn’t come easy,” he wrote.
He also said he was “praying it’s the last time I have to come back.”
His family rallied around him.

Kaia called him a “warrior,” while Cindy wrote that he was a “warrior” who had “walked thru the fire to find yourself.”
Six months later, Presley entered a rehabilitation facility in California.
And on September 20, his family was faced with the unimaginable.
Presley was dead. He was only 27.
