Nobody outside her family knows, and her family hasn’t said.
What we have is her own remark on the second season premiere of The Real Housewives of Miami: a surgeon had ruined her. That’s it. No clinic, no date, no procedure.
The surgeons quoted in all the coverage never treated her and had no records to work from. One of the most widely cited analyses is perfectly open about this — it’s a reading of published photographs, describing what the author believed had been over-augmented. An opinion about an image, presented for ten years as a finding.
So if you came here for the definitive answer, the honest one is that there isn’t one. And it’s worth knowing that rather than being handed somebody’s guess.
What did happen to Elsa Patton — medically?
Something serious did, and hardly anybody writes about it.
In March 2013 she suffered a stroke and fell, and blood accumulated on her brain. Marysol described it at the time: “The doctors informed me that they needed to operate on my mother’s brain immediately to relieve the pressure of blood that was accumulated and causing swelling.”
She had emergency brain surgery.
And then she went back and filmed season three.
How did Elsa Patton die?
She died over Mother’s Day weekend in 2019, aged 84. The news was announced on 12 May.
No cause was ever made public. The family’s statement said only: “Our beloved Elsa Patton (AKA Mama Elsa) passed away over Mother’s Day weekend after a long illness. She was surrounded by her family and close friends.”
And then they added a line that tells you more about who she was than any surgical analysis ever could: “In lieu of tears the family asks you to raise a glass of good champagne in honor of Elsa’s memory.”
A private ceremony was held in Miami on 17 May 2019. Her husband Donald Patton, to whom she’d been married 50 years, had died roughly fifteen months earlier, in February 2018.

Cuban-born, Miami-based, the mother of Real Housewives of Miami cast member Marysol Patton — and, by more or less universal agreement, the most quotable person on a show she was never actually cast in.
She appeared across all three original Bravo seasons, from 2011 to 2013. Bravo’s own tribute credited her “hilarious one-liners, iconic style, and strong spirituality,” which is about right.
She’s survived by Marysol, her son Thomas Anthony Jones, daughter-in-law Svetlana and grandson Hunter.

Bravo released nine episodes simultaneously on 17 September 2012, following her as she launched a Cuban coffee brand, also called Havana Elsa. Developing the line. Promoting it around Miami. Landing her first customer.
